I love my dentist. He does excellent work, and the office staff is always pleasant and helpful. He is more expensive--the "usual and customary fees" that my insurance company pays are nowhere near the prices he charges (but where do they get that usual and customary fee schedule from anyway?)
The only drawback is that he doesn't discuss the costs associated with his work unless you specifically ask him in advance. As he's discussing the treatment, he tells you everything he's going to do, explains the process, etc., but he doesn't mention any of his fees, so if you're shy about asking, you may be in for a shock when you get the bill.
